Narrative Summary
On 01/10/20 at approximately 13:33 PM, OSP was dispatched to a report of a driving complaint on SR-18 near milepost 12. According to the information received, a maroon minivan was westbound from that area and was swerving all over the roadway. The same vehicle was reported to have then been near the intersection of Slick Rock Creek Road and SR-18. The vehicle was reported to have been unable to maintain its lane and had crossed the centerline, nearly causing a head on collision. The vehicle was located turning into a driveway located at 3047 Salmon River Highway and a stop for reasonable suspicion of driving under the influence was conducted. On contact the driver appeared to be extremely visibly intoxicated. He did not know where he was and his speech was heavily slurred. The driver was identified with an Oregon ID card. I could smell a strong odor of alcoholic beverage coming from his person. His eyes were heavily bloodshot and glassy. He appeared to be highly confused, and lethargic. A check of his driving status through LEDS/NCIC showed he was Suspended/Revoked – Felony level. The driver was also found to be on felony probation in Lincoln County County for DUII- Felony., with no alcohol conditions. The driver stepped from the vehicle on his own accord and was unsteady on his feet. He was advised of his Miranda Rights. When asked if he would conduct SFST’s, he refused. He was provided the Rohr’s admonishment for the tests, and still continued to refuse. He was taken into custody for DUII and Felony DWS without incident at 13:55 PM. An empty alcoholic seltzer water was found sitting next to the driver’s seat. Quackenbush was transported to the Lincoln City Police Department where he was provided opportunity to provide a breath test under Implied Consent. He refused to provide a test under Implied Consent. The driver later provided a breath tests which showed a blood alcohol level of 0.23 percent by weight. The operator had three previous DUII convictions within the last ten years. PO Ken Krause was contacted and also placed a detention warrant on the driver. He was transported to the Lincoln County Jail where he was lodged DWS Felony and DUII

***LOG ONLY*** On January 10, 2020 the Oregon State Police tagged a reported abandoned vehicle at the Boiler Bay State Park. The vehicle did not have license plates on it. UNIT 1 (blue Kia sedan) was found still at the location on January 13, 2020 and a tow was requested to remove the abandoned vehicle. Contacted was attempted with registered owner but found to be unsuccessful. Car Care Towing responded to perform this request.

***E-CRASH*** On the evening of January 10, 2020, the Oregon State Police responded to single vehicle non-injury traffic crash on Hwy 20 near mile post 30. The crash investigation found UNIT 1 (blue Scion coupe) lost control on the roadway for unknown reasons after the driver reportedly had passed another vehicle. UNIT 1 struck and damaged the east bound guard rail and came to rest blocking both east bound lanes of the Highway. The driver was cited for Failure To Drive Within Lane. The driver made his own arrangements through Bob’s Towing out of Corvallis to remove the vehicle. OSP was assisted with the investigation by the Lincoln County Sheriff’s Office.

Narrative Summary
**eCrash Report** On 01/11/2020 at approximately 05:45 AM, Unit #1, a blue 2005 Dodge medical transport van, was eastbound on US Highway 34 near milepost 5. The area was dark and unlit during a heavy rain and windstorm. A small landslide occurred along the south side of the highway, and a large tree fell across the highway. The operator of the van did not see the tree due to oncoming vehicle headlights and crashed head on into the tree. The van sustained heavy front end damage. The van came to rest along the southern shoulder of the roadway. The driver was wearing his seatbelt and the front airbags deployed. No injuries were reported. Crews from ODOT Incident response removed the tree and landslide debris from the roadway. The vehicle was later towed at the owner’s request by AA Rowley’s Towing of Newport.

(ecrash) On 1/12/19 at approximately 11:46 pm I was dispatched to a crash with injuries which occurred on SR 18 near mile post 9. Unit #1, a black GMC Suburban (CA53680), was eastbound as it approached two vehicles westbound. The driver of Unit #1 reported the rear westbound vehicle began an unsafe pass, forcing him to swerve off the eastbound shoulder to avoid a head-on collision. Unit #1 continued off the eastbound shoulder and struck a large tree, head-on a few feet down the embankment. Three passengers sustained non-life threatening injuries in the crash and were transported by ambulance to North Lincoln Hospital for their injuries. Neither the driver nor any passengers could provide any descriptive characteristics of the other vehicle involved in the crash. Unit #1 was recovered and towed from the scene by Car Care Towing.

***E-Crash*** On the afternoon of January 13, 2020, the Oregon State Police responded to a two vehicle non-injury, non-blocking traffic crash on Highway 101 near mile post 127 in Depoe Bay, OR. The crash investigation found UNIT #1 (blue Jaguar SUV) failed to yield to the right of way when it turned left (south) out of a parking lot onto Hwy 101 and struck UNIT #2 (gray Honda mini-van). The driver of UNIT #1 over-corrected after crashing into UNIT #2 and struck a street sign. The driver of UNIT #1 was cited for Fail to Yield before Entering Hwy. Both driver’s made their own arrangements to have their vehicles towed from the scene. UNIT #1 was towed by Lincoln City Towing. UNIT #2 was towed by Rowley’s Towing.

(ecrash) On 1/14/20 at approximately 5:41 pm I was dispatched to a vehicle down an embankment on SR 229 near mile post 27. Upon arrival I determined it was not a reportable crash and requested a tow responded to assist the driver back onto the roadway. Coast Towing responded to assist. While attempting to pull the vehicle (Tan Ford Thunderbird VMS119) back on the highway the tow truck significantly damaged the vehicle, rendering it unserviceable. A crash investigation was completed. Coast Towing towed the vehicle from the scene. OSP and ODOT provided the driver a courtesy transport home.
